PREPARING FOR YOUR VIRTUAL VISIT AT MEISTER ORTHODONTICS

We can’t wait to meet you!

 What you will need:

1. Please complete our patient forms including, either, the Child or Adult Acquaintance Record, the Verbal Authorization Form, and the HIPAA Notification .
2. Your smart phone
3. Your removable appliance(s), if applicable. *Please take one set of photos with your appliance(s) IN and one with your appliance(s) OUT.

What you will need:

• A family member, friend or tripod
• A cell phone with a camera
• Two plastic or metal spoons

How to take your smile photos

 
Have a family member or friend take 8 key photos.

Please include the photos displayed below.

Just a few tips on how to get the best photos possible: 

• Turn your camera phone to the left for landscape view for all of the photos (your phone should be horizontal with the camera lens on the upper left)
• Good lighting helps
• Use a white or light background
• Pull your hair back if you have long hair and tuck hair behind the ear for the side photos if they are hidden
• Remove any jewelry that may be visible in the photos

For the next series of photos you will need your spoons! Using the spoons, retract your cheeks as demonstrated in the photos below.
